Southern Oregon Case mix: Pediatric Dental and/or Adult Dental, with exposure to multi-specialty office-based settings (GI, ENT, gynecology/fertility, plastics) Employment: W2, 0.7–1.0 FTE Portland, OR Case mix: Adult Dental and/or multi-specialty office-based environments (GI, ENT, gynecology/fertility, plastics) Employment: W2, 0.7–1.0 FTE Springfield | Corvallis | Roseburg | Salem, OR Case mix: Pediatric Dental Employment: W2, 0.8–1.0 FTE Schedule & Practice Model Cases typically begin between 6:00–7:00 AM Most days conclude by 12:00–1:00 PM Fully independent, office-based anesthesia model (no ASC or hospital work) Clinical Setup Providers bring and manage their own anesthesia equipment Monthly replenishment via Group-provided drug and supply kits Select locations include Dental Anesthesia Assistants (site-specific; details reviewed during screening) Malpractice Coverage – W2 Provider Covered under Group-sponsored policy (PPM) Claims-made policy, with premiums fully covered by Group Standard limits: $1M / $3M Tail coverage: Not included by default Optional “Perma-tail” available through PPM (converts to occurrence-based) If selected, provider is responsible for ~6% premium increase Compensation & Benefits Compensation varies by location and FTE (.7–1.0) Comprehensive W2 package includes: Medical benefits 401(k) 6 paid holidays + 21 days PTO Additional Incentives Retention bonus available Finalized post-initial screening and tied to overall contract structure Determined in partnership with Finance and Operations Patient Population Predominantly healthy pediatric patients (ages 5–17) for dental procedures (extractions, fillings) Adult cases primarily ASA I across supported specialties
